Presbyterian Homes of Georgia currently has an exciting opportunity for a dynamic and experienced *Licensed Practical Nurse or Registered Nurse* to serve as the *MDS Coordinator* at Presbyterian Home & Retirement Community in Quitman, GA.
The *MDS Coordinator* will coordinate and ensure completion of the state required Minimum Data Set on the residents in the Skilled / Long-Term Care section to include admissions, significant changes, quarterly and Medicare assessments in a timely manner.
They will identify resident problems from the MDS and other assessments and develop the initial individual Care Plan for each resident along with the other disciplinary team members. The *MDS Coordinator* reviews and optimizes the MDS Process to ensure appropriate services are rendered that appropriately justifies reimbursement.
*Responsibilities (overview):*
* Coordinates and ensures completion of the Minimum Data Set (MDS) on skilled / long-term care residents.
* Initiates and actively participates in Interdisciplinary Care Plan meetings.
* Coordinates the development and revision of resident Interdisciplinary Care Plans based on information identified in the Resident Assessment Instrument/Minimum Data Set (RAI/MDS).
* Ensures the Interdisciplinary Care Plan addresses all triggered indicators.
* Collaborates with nursing caregivers and other disciplines to obtain information on residents' status and progress toward care plan goals.
* Demonstrates the ability to communicate effectively with a variety of other disciplines to ensure the accurate and timely completion of the RAI/MDS for transmission.
* Prepares accurate reports of the Quality Measure/Quality Indicators (QM/QI) derived from the RAI/MDS which drives the data for the CLC Compare. Performs tracks and trends of this data to enhance the knowledge of the Interdisciplinary Team and improve quality of care through performance improvement initiatives.
* Active involvement in Quality Improvement.
* Implements an educational plan to meet changing program or service needs.
* Actively participates in delivering hands-on nursing care and services on the Nursing Unit(s).
The *MDS Coordinator* will need to be a highly engaged, compassionate and knowledgeable RN or LPN who embraces and promotes person-centered care delivery models and can actively engage with the leadership team, staff, residents, family members and the community as a whole.
The successful candidate will be highly collaborative, and partner successfully with the Nursing Administration, as well as other members of the campus disciplinary/leadership team to ensure operational and service excellence in nursing care.
As an important member of the Nursing team, the MDS Coordinator will help to implement Presbyterian Homes of Georgia’s ongoing philosophy and strategy as a Christian based organization that provides exceptional care and services for senior adults.
The ideal applicant *should* have the following minimum qualifications:
* A Caring Heart and a Passion for working with Senior Adults.
* Current Georgia Nursing Licensure, LPN or RN (preferred)
* Minimum of one (1) year MDS Coordinator experience in the skilled / long-term care setting.
* Have a current/active CPR certification.
* Experience in clinical and utilization experience in a Long Term Care or skilled facility.
* Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to skilled nursing/ long-term care.
* Strong working knowledge of Medicare, Medicaid reimbursement guidelines, PPS and Rugs categories as it relates to the MDS process.
* Personal integrity and professionalism to work effectively with the Interdisciplinary Team, Patients, and Families.
* Strong & solid knowledge of both Skilled Nursing and Assisted Living regulations.
* Experience in dementia, Alzheimer's, memory support is a plus.
* Proven ability to engage with employees at all levels in a positive, productive manner.
* Excellent interpersonal skills.
* Analytical & problem-solving skills.
* Strong ability to manage, multi-task and communicate effectively.
* Proficiency in Microsoft Office software; EMR / EHR software.
* Having multiple levels of nursing experience in Skilled Nursing, acute hospital or similar healthcare settings is strongly preferred.
